It is the policy of Lumos Youth Counseling not to discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), age, disability, or sex. This is our grievance procedure for the prompt and equitable resolution of complaints alleging any action prohibited by Section 1557 of the Affordable Care Act and its implementing regulations at 45 CFR part 92, or by Section 504 of the Rehabilitation Act of 1973 and its implementing regulations at 45 CFR part 84, issued by the U.S. Department of Health and Human Services.
Anyone who believes that Lumos Youth Counseling subjected someone to discrimination prohibited by these laws may file a grievance under this procedure.

Contact our Civil Rights Coordinator in any of these ways:
- In person or by mail: 14241 South Redwood Rd Suite 300, Building A, Bluffdale, Utah 84065

When to file and what to include
- File your grievance within 60 days of the date you become aware of the alleged discriminatory action.
- A grievance should generally be in writing and include the name and contact information of the person filing it, the alleged discriminatory action, the alleged basis (or bases) of discrimination, the date the grievance was filed, and any other pertinent information.
Investigation and decision
- When a grievance includes allegations that would violate Section 1557 or Section 504, the Civil Rights Coordinator (or their designee) will promptly investigate it. - Lumos Youth Counseling will issue a written decision to the person who filed the grievance no later than 30 days after it is filed. The decision will include the resolution date and a notice of the right to pursue further administrative or legal remedies.

Confidentiality and records
The Civil Rights Coordinator will keep confidential the identity of anyone who files a grievance, except as required by law or to carry out the purposes of this procedure, including to investigate the grievance. Lumos Youth Counseling keeps grievance files and records confidential and retains them for at least three years from the date the grievance is resolved.
No retaliation
It is against the law for Lumos Youth Counseling to intimidate, threaten, coerce, retaliate against, or otherwise discriminate against anyone who files a grievance or participates in the investigation of a grievance.
Help to take part
Lumos Youth Counseling will make appropriate arrangements so that people with disabilities and people with limited English proficiency receive reasonable modifications, appropriate auxiliary aids and services, or language assistance services, if needed to take part in this grievance process. These services are provided in a timely manner and free of charge.
